Inductiebehandeling: 1e gift altijd normale dosis, vervolgens:
kreatinineklaring 40–59 ml/min: 450 mg 2×/dag;
25–39 ml/min: 450 mg 1 ×/dag;
10–24 ml/min: 450 mg om de dag.

Onderhoudsbehandeling, of profylaxe:
kreatinineklaring 40–59 ml/min: 450 mg 1 ×/dag;
25–39 ml/min: 450 mg om de dag;
10–24 ml/min: 450 mg 2 ×/week.

Toediening bij een kreatinineklaring < 10 ml/min wordt niet aanbevolen.

Hemo dialysis: inductiebehandeling: 200 mg na elke dialyse, eerste gift normale dosis. Onderhoudsbehandeling: 100 mg na elke dialyse.
